For more information, please visit www.minds.org.sg. RESPONSIBILITIES The Life Coach (ELP) is to provide high quality of daily care to the ELP residents clients. He/She is to supervise and to ensure the well-being of the residents/ clients with intellectual disability and/ or autism. He/She is also to provide training to enhance the development of the residents/ clients in the area of independent living skills, leisure, recreation, sports and career development and participation in the community. Support ELP Residents • Participate in screening interviews to assess prospective residents' suitability for the ELP program. • Provide comprehensive pre-onboarding, onboarding, and post-onboarding support to facilitate a seamless transition for clients moving into ELP units • Provide consistent guidance to help residents/clients manage independent living in ELP onsite units. • Facilitate community support networks and organize social/recreational activities. • Conduct regular visits to ensure housemates maintain a harmonious living dynamic. • Serve as the primary contact for conflicts, emergencies, and assistance requests. Case Stewardship • Maintain accurate records, care plans, and incident reports. • Ensure compliance with medical requirements and dietary restrictions. • Ensure the living environment is safe, clean, and well-maintained. • Proactively track the health and hygiene status of residents/clients. • Prepare, monitor, and review Individual Care Plans (ICPs). • Assist in the design and periodic review of operational SOPs to ensure efficiency. • Support the Manager in the compilation and submission of monthly, 6-monthly, and annual KPI reports. • Assist the SGE Research team in facilitating the biannual Quality of Life (QoL) surveys. Capacity Building (Skills) • Conduct Life Planning sessions and regular reviews to ensure all training goals are aligned with the client's personal aspirations. • Assess individual strengths and needs to tailor training toward resident and caregiver-identified goals. • Coach and mentor residents in living skills, leisure, sports, and career/personal development. • Reinforce existing competencies while identifying and scaffolding opportunities for further growth. Capacity Building (Safety & Environment) • Develop and implement training programs in collaboration with internal team members and AHPs. • Ensure the reliable operation of monitoring systems and emergency alert devices as part of the safety framework. • Identify potential hazards and implement proactive strategies to mitigate risks within the living environment. Case Work • Participate in Multi-Disciplinary Team (MDT) case reviews to evaluate resident progress and refine support strategies. • Engage regularly with caregivers and social workers to align on family-centred goals and long-term future care planning. Community Integration • Build and sustain rapport with families, employers, grassroots partners, and volunteers/befrienders. • Mentor residents in navigating public resources, managing personal finances, and ensuring safe travel to work and activities. • Act as a resource bridge, connecting residents to external social support services and community programs. QUALIFICATIONS • Min. Diploma in any discipline • Min 1 year of working experience • Knowledge and experience in residential, assisted living, home based services, or disability service is an added advantage. • Skills in planning and implementing community activity and training programmes. • Proficient in Computer literacy skills - MS Words, Excel and PowerPoint • Good interpersonal and communication skills and a good team player OTHER INFORMATION • Work location: Walking distance from Redhill MRT • Able to commit to 5 days work week rotating shifts (morning shift/ afternoon shift), alternate sat/ sun and public holidays.
