Locum Staff Nurse, Home Care (office hours/ $65/ case) #HGN

Job scope:  • Clinical Assessments & Planning: Conduct holistic nursing assessments on homebound patients, create and update individualized care plans, and monitor patients' vital signs and overall health conditions. • Nursing Procedures: Perform skilled procedures such as wound dressing/management (including Negative Pressure Wound Therapy), urinary catheter change (IDC), nasogastric tube feeding/insertion (NGT), stoma care, injections, and phlebotomy. • Medication Management: Administer, manage, and pack medications, including oral, injections, and managing intravenous medication. • Caregiver Education & Training: Educate patients and caregivers on care strategies, infection control, and proper use of medical equipment to manage health conditions at home. • Coordination & Reporting: Liaise with hospital teams, doctors, and community partners to coordinate patient care, as well as document visit observations and report changes in conditions. • Safety & Compliance: Ensure infection control and safety measures are followed during visits and maintain accurate medical records.
