About this role
Job Description: • Dispensing and educating patients on their medications, medication review and reconciliation, therapeutic interventions and recommendations, multidisciplinary rounds and collaborative prescribing for qualified persons. • Recommend and monitor drug therapy to achieve desired outcomes. • Provide drug information to other healthcare professional and patients. • Identify, resolve and prevent drug-related problems. • Provide pharmaceutical care, working collaboratively in a multidisciplinary team in various clinical settings, including outpatient, inpatient and community settings. • Ensure pharmacy policies and procedures are implemented consistently. • Other ad hoc duties assigned Interested candidates do attach your resume to kate.leau@recruitexpress.com.sg Attn: Kate Leau Wei Xuan (R1552331) Recruit Express Pte Ltd (99C4599)
