About this role
Case & Family Management • Provide end-to-end case management support, including comprehensive assessment, care planning, and the implementation of appropriate interventions. • Conduct psycho-social assessments and perform home visits when necessary to gain a holistic understanding of the student's needs and family environment. • Offer individual counselling and practical assistance to students and families to improve coping mechanisms, resilience and self-esteem. • Review casework interventions to ensure effectiveness, and close cases upon achieving sustainable service outcomes for families. • Maintain accurate case notes, documentation, and prepare required social reports in accordance with professional standards Financial & Resource Navigation • Conduct means-testing and process applications for financial assistance schemes in accordance with the school’s policies and guidelines. • Act as a crisis intervention and support resource for families during times of significant need. • Secure and facilitate the provision of financial subsidies and appropriate community resources for families. Admissions & Intake Coordination • Participate as a member of the Admissions team to process incoming student applications and coordinate required documentation. • Conduct post-admission parent interviews to assess family coping, identify presenting needs, and provide early interventions when necessary. • Liaise with parents, feeder schools, and relevant agencies to support the application and intake process. Family and Group Support Services • Plan, facilitate, and evaluate parent/stakeholder group engagement sessions focused on sharing knowledge, building social support systems, and creating resilient networks. • Actively involve parents and caregivers in school-based programmes and activities to strengthen the home–school partnership and extend outreach efforts. Programme Development & Community Work • Support programme development through data collection, documentation, and basic data analysis. • Assist in the implementation and promotion of programmes to target client groups, stakeholders, partners, and external agencies. • Conduct environmental scans and assess broader community needs through data-gathering and analysis to inform programme planning and continuous improvement. Job Holder Requirements and Attributes: • Possess a recognised degree and/or graduate diploma in Social Work • Preferably with 2 – 4 years of relevant professional experience • Proactive and self-motivated work attitude with strong interpersonal, communication, and organisational skills. • A strong team player who can work effectively and harmoniously within a multidisciplinary team setting. • Strong capacity to work independently and handle pressure while managing a diverse caseload effectively. • Experience working with children/adults with special needs especially those with Autism Spectrum Disorder (ASD) is an added advantage. • For Senior Social Worker roles: Ability to provide clinical supervision to staff and interns.
