Home Care Doctor

About the roleOur client is a well-established community hospital in Singapore committed to integrated, holistic care for patients with chronic and complex conditions. They are seeking dedicated medical doctors to join their Home Care Services team. You will deliver compassionate, person-centred medical care to patients in community and home-based settings — supporting integrated, longitudinal care across the full care continuum. This role is open to candidates at various stages of their medical career, from Medical Officer level through to Consultant. What you'll do• Conduct home visits, teleconsultations, and follow-up reviews for patients under ambulatory Home Care Services • Participate in multidisciplinary case discussions, transitional care planning, and integrated care coordination • Maintain accurate clinical documentation and contribute to quality improvement and audit activities • Support teaching, mentoring, and supervision of junior staff where applicable • Support continuity of care across outpatient, home care, nursing home, and community-based settings as required What we're looking for• Basic medical degree with full registration to practise in Singapore (MCR required) • Postgraduate qualification in Family Medicine, Geriatric Medicine, Internal Medicine, or Palliative Medicine preferred (for senior roles) • Experience in community, home-based, intermediate, or chronic care is advantageous • Genuine commitment to person-centred, integrated, and community-focused care
