About this role
- Manage and follow up all-new e-referrals from AIC. - Client initial assessment for ADLs, functional, environmental psycho-socialand health needs. - Monitors and verifies services each client receives and evaluates theeffectiveness of each intervention. - Manage cases and provide family counselling services. - Conduct interviews to determine client eligibility. - Supervise all claims eg. Subvention & SMF. - Supervise application of Senior Mobility Funds. - Seek alternative funds to support the clients' financial assistance.
