About the Role Looking for a meaningful role where you can make a difference in people's lives every day? As an Assistant/Care Coordinator, you'll work closely with patients and caregivers to help them better understand their health, stay on track with screenings and vaccinations, and connect them with healthcare and community resources. This role combines patient engagement, health education, and care coordination, making it ideal for individuals who enjoy interacting with people and supporting them on their healthcare journey. You'll gain valuable exposure to preventive healthcare, community care, and patient support while working alongside healthcare professionals in a collaborative environment. Key Responsibilities • Build rapport with patients and caregivers, and provide support throughout their healthcare journey. • Encourage patients to attend health screenings, vaccinations, and follow-up appointments. • Provide basic health education on topics such as healthy eating, exercise, chronic disease management, and preventive care. • Teach patients how to use home health devices such as blood pressure monitors, weighing scales, and glucose monitors. • Help patients access healthcare services, community programmes, and support resources. • Record patient information and update care activities using healthcare systems. • Assist with basic health measurements and monitoring activities. • Work closely with healthcare professionals and community partners to support patients' overall well-being. • Support administrative and coordination activities to ensure smooth patient care delivery. Requirements • Min. O / A Levels / NITEC / Diploma • Prior experience in healthcare, customer service, community care, social services, or patient-facing roles is an advantage. • Enjoy interacting with people and helping others. • Good communication and interpersonal skills. • Comfortable using computers and Microsoft Office applications. • Able to work both independently and as part of a team. • Willing to travel between healthcare facilities and work on Saturdays as part of a 5.5-day work week.
