Duties & Responsibilities: • Conduct comprehensive biopsychosocial assessments to understand residents’ social support, mental health needs, coping abilities, and environmental factors. • Develop individualized care plans in collaboration with residents, families, and multidisciplinary teams, with measurable goals and tailored interventions. • Provide supportive counselling to residents and their families, addressing emotional needs, concerns, grief, and adjustment issues. • Identify and coordinate internal and external resources to meet residents’ psychosocial, emotional, and practical needs. • Liaise with community partners, SSAs, hospitals, and government agencies to ensure appropriate support. • Participate actively in discharge planning meetings, assisting residents and families with transitions to community living, step-down care, or long-term arrangements. • Maintain accurate and timely documentation of assessments, care plans, progress notes, and case updates in compliance with regulatory and audit standards. • Provide guidance to families on coping strategies, available resources, advance care planning, and end-of-life considerations. • Maintain up-to-date knowledge of geriatric care, counselling practices, social service frameworks, and case management standards. • Support Home-wide programmes, initiatives, and community engagements. • Perform any other duties as assigned by the Social Work Supervisor or Head of Home. Requirements: • Bachelor’s Degree in Psychology, Counselling, or related field. • Relevant experience in healthcare, eldercare, or geriatric social service settings preferred. • Proficient in psychosocial assessment tools, counselling techniques, and case management approaches. • Strong interpersonal skills with the ability to build rapport with elderly residents and their families. • Skilled in crisis intervention, problem-solving, and advocacy. • Familiarity with Singapore’s social service ecosystem, community resources, and support networks. • Commitment to ethical practice, confidentiality, cultural sensitivity, and diversity. • Demonstrates alignment with Home’s mission, vision, and service values.
